Preparation checklist Latest four statement periods
Have ready: Business and funding details, principal owner information, and your latest four business bank statement periods.
Business records
Legal name, entity type, EIN, start date, industry, location, revenue, and employee count. Have the business address and state, monthly revenue, and annual revenue range in view. DBA, website, and months in business are optional.
Funding needs
The amount requested, when funds are needed, preferred remittance frequency, and intended use. Explain the business purpose and how you arrived at the amount. These preferences do not set offer terms.
Principal owner details
Name, ownership percentage, email, US phone number, date of birth, SSN or ITIN, self-reported credit, and home address. Add the principal owner. If ownership is below 100%, you can optionally add other owners. Complete the details for each owner you include.
Your latest four statement periods
Prepare business bank statements covering the most recent four statement periods. One PDF can cover several periods, and one period can be split across files. Four files alone do not establish four-period coverage.
